Our Focus

In 2012, San Francisco’s nonprofit hospitals, San Francisco Department of Public Health, and UCSF came together in collaboration with community residents and stakeholders to create a Community Health Improvement Plan (CHIP) for San Francisco.

Safe and Healthy Living Environments

Our goal is to improve safety and crime prevention, reduce exposure to environmental hazards, and foster safe, green “active” public spaces.

Healthy Eating and Physical Activity

Our goal is to increase healthy eating and physical activity among residents by creating environments that make healthy choices the easy choices.

Access to Quality Health Care and Services

Our goal is to improve the integration and coordination of services, increase connection of individuals to the health services they need, ensure services are culturally and linguistically appropriate and that residents have access to a health care home.
